Background:Incontrasttotheestablishedefficacyofanti-tumornecrosisfactorα(TNFα)onBMEofthesacroiliacjoints(SIJ)(andspine),surprisinglylittleinformationexistsontheimpactofNSAIDs.AsNSAIDtherapywasalreadythecornerstoneoftreatmentbeforemagneticresonanceimagingoftheSIJwasintroduced,verylimiteddataontheeffectofoptimalNSAIDtherapyontheextentofinflammation,quantifiedbyavalidatedscoresuchastheSpondyloarthritisResearchConsortiumofCanada(SPARCC)scoringsystem,hasbeenpublished.OurgoalwasthereforetoexaminetheimpactofNSAIDuseondiseaseactivityandBMEinaninceptioncohortofnewonsetAxSpA.
Objectives:ToevaluatetheimpactoffulldoseNSAIDsontheextentandintensityofbonemarrowedemaofthesacroiliacjointsonmagneticresonanceimaginginaxialspondyloarthritis(AxSpA).
Methods:Asinglecenter,6-weekstudyinaconsecutivecohortofpatientswithclinicalsuspicionofAxSpA.Atotalofpatientswerescreened.ThirtysixpatientswithdiagnosisofAxSpAandpositiveMRISIJwereincludedforafollow-upMRIafter6weeksofoptimaldoseofNSAIDs.DiseaseactivitywasmonitoredbyaBASDAIquestionnaireevery2weeksandBASFIatbaselineandweek6.NSAIDintakewasassessedbytheASASNSAID-index.PrimaryendpointswereBASDAIresponseandimprovementoftheSpondyloarthritisResearchConsortiumofCanada(SPARCC)scoreofMRISIJatweek6.
