Abstract
OBJECTIVES:
ToreviewandupdatetheexistingdefinitionofapositiveMRIforclassificationofaxialspondyloarthritis(SpA).
METHODS:
TheAssessmentinSpondyloArthritisInternationalSociety(ASAS)MRIworkinggroupconductedaconsensusexercisetoreviewthedefinitionofapositiveMRIforinclusionintheASASclassificationcriteriaofaxialSpA.ExistingdefinitionsandnewdatarelevanttotheMRIdiagnosisandclassificationofsacroiliitisandspondylitisinaxialSpA,publishedsincetheASASdefinitionfirstappearedinprintin,werereviewedanddiscussed.TheprecisewordingoftheexistingdefinitionwasexaminedindetailandthedataandadraftproposalwerepresentedtoandvotedonbytheASASmembership.
RESULTS:
TheclearpresenceofbonemarrowoedemaonMRIinsubchondralboneisstillconsideredtobethedefiningobservationthatdeterminesthepresenceofactivesacroiliitis.StructuraldamagelesionsseenonMRImaycontributetoadecisionbytheobserverthatinflammatorylesionsaregenuinelyduetoSpAbutarenotrequiredtomeetthedefinition.Theexistingdefinitionwasclarifiedaddingguidelinesandimagestoassistintheapplicationofthedefinition.
CONCLUSION:
ThedefinitionofapositiveMRIforclassificationofaxialSpAshouldcontinuetoprimarilydependontheimagingfeaturesofactivesacroiliitisuntilmoredataareavailableregardingMRIfeaturesofstructuraldamageinthesacroiliacjointandMRIfeaturesinthespineandtheirutilitywhenusedforclassificationpurposes.
